Accurate Information
Your healthcare provider has reviewed your medical condition and made a clinical treatment decision based on the information you provided. If you forgot or incorrectly provided any health-related information, the provider may misdiagnose or fail to diagnose conditions that could affect their treatment recommendation. If you need to clarify or update any health information, please message your provider team through the LeanRx platform as soon as possible.
Treatment Failure
This treatment is not 100% effective for all patients. It may not work or may only partially resolve the condition for which you are seeking treatment. If you do not see any improvement or if you experience new or worsening symptoms, please message a provider through the LeanRx platform at any time. You can also contact your regular healthcare provider, go to an urgent care center, or call 911 if it is an emergency.
Misdiagnosis or Delayed Diagnosis
There is also a risk that the provider may misdiagnose or fail to diagnose conditions that you may have, which could affect the treatment recommendation. The healthcare providers providing treatment through the LeanRx platform use evidence-based guidelines and clinical decision-making to try to minimize these risks.
Accepting or Declining the Recommendation
The provider who recommended this treatment for you did so with your consent and because they believe that the potential benefits outweigh the risks. Please evaluate this information and any input from your in-person healthcare team, and any other relevant information to decide if this treatment plan is appropriate for you. You are free to not follow the treatment recommendations you received from the provider through the LeanRx platform.
Share with Your In-Person Healthcare Team
You can request a copy of your health records at any time. We strongly recommend that you inform your in-person healthcare team about any new medications you are taking or other changes in your health. You can also ask questions at any time by messaging a provider through the LeanRx platform. They can help you share any information about your health with your in-person healthcare team.
